“England’s GPs Shift Focus to Job Coaches for Sick Notes”

General practitioners (GPs) in England have been instructed to cease the issuance of millions of sick notes and instead refer patients to job coaches or fitness facilities. Health Secretary Wes Streeting emphasized a shift in England’s approach, moving away from merely treating illnesses following a substantial increase in sick leave cases over the past decade.

Under the new initiative called WorkWell, GP practices have received funding to offer specialized support services such as career coaching, physiotherapy, and occupational therapy to patients, rather than solely signing them off work. This marks the most significant alteration to the “fit note” system since its inception in 2010, aiming to address the root causes of health issues hindering people from leading healthier lives.

The surge in fit notes issued by GPs, now totaling 11 million annually compared to five million a decade ago, prompted the reform. Data indicates that GPs in England are increasingly suggesting adaptations for individuals to either remain in or return to work within the fit notes they provide.

Streeting highlighted the importance of moving beyond a paper-based approach to managing illnesses, advocating for practical solutions through the WorkWell program. This initiative connects individuals receiving fit notes with tangible support services like physiotherapy, mental health assistance, and guidance on workplace adjustments.

Furthermore, the WorkWell scheme, initially piloted across 15 regions with high unemployment rates and sickness levels, has now been expanded nationwide. Family doctors will collaborate with employment coaches to assist patients in resume writing, and social prescribing link workers will refer individuals to resources like gym memberships and gardening classes. Charities offering support with financial management and housing issues will also be involved in the process.

Recent data indicates a slight decrease in the number of fit notes issued, suggesting early signs of progress in addressing the underlying causes of absences from work. Streeting emphasized the need for ongoing reforms to ensure that the fit note system effectively supports recovery and facilitates a smooth return to work.

Fit notes, introduced in 2010 to replace sick notes, have faced criticism for their similar usage patterns. These notes can last up to three months during the initial six months of absence from work, with a review date, and can be extended for longer periods thereafter.

In support of the WorkWell initiative, Work and Pensions Secretary Pat McFadden emphasized the importance of providing individuals with the necessary support to maintain their health and employment status, benefiting both individuals and the economy.
