Shocking recent video footage depicts the tragic moment a plane ignited into flames during a fatal accident that claimed the lives of 15 individuals. The video has surfaced as The National Transportation Safety Board conducts an inquiry into the cause of the tragic incident that transpired on November 4, 2025, in Kentucky.
In the distressing event, a UPS jet’s engine and pylon detached from the wing of the McDonnell Douglas MD-11F aircraft during takeoff at Louisville Muhammad Ali International Airport. This led to the fatalities of three crew members and 12 individuals on the ground. The aircraft, which was 34 years old at the time, was en route to Hawaii.
The footage captures the harrowing moment when a blaze engulfs the airplane, leaving behind a significant debris field. Following the crash, all MD-11s were grounded by the FAA for several months.
The NTSB is currently conducting a hearing in Washington to investigate the circumstances surrounding the collision. Among the victims on the ground were identified individuals like Angela Anderson, Carlos Fernandez, Trinadette “Trina” Chavez, Tony Crain, John Loucks, John Spray, Matthew Sweets, Ella Petty Whorton, Megan Washburn, Louisnes Fedon, and his 3-year-old granddaughter Kimberly Asa. Additionally, Alain Rodriguez Colina, who sustained severe injuries in the incident, passed away on Christmas Day.
UPS confirmed that its employees Capt. Richard Wartenberg, First Officer Lee Truitt, and International Relief Officer Capt. Dana Diamond were the three crew members onboard the ill-fated flight. Reports indicated that the plane was carrying 38,000 gallons (144,000 liters) of fuel at the time of the crash.
Aviation attorney Pablo Rojas remarked, “Really the plane itself is almost acting like a bomb because of the amount of fuel.” The fire engulfed the massive aircraft and extended to nearby establishments. Firefighters worked tirelessly throughout the night to combat the blaze over a vast area and conducted searches for potential victims.
