Angela Boorn found her 75-year-old uncle, Gerald McClellan, deceased in a chair at the West Valley assisted living facility, as confirmed by authorities in an interview with Phoenix CBS affiliate KPHO. McClellan had been unnoticed for at least three to four days.
Boorn, who considered her uncle like a second father, expressed shock and devastation at the lack of attention given to him. She described the scene as seeing a skeletal figure in the chair with hollowed-out eyes. Despite her routine visits and calls, she received no response from McClellan on October 16.
After discovering her uncle’s condition, Boorn immediately sought answers from the facility staff. The executive director informed her that no one had checked on McClellan since the previous weekend. Boorn had placed McClellan in the LifeStream facility at Sun City in January after the passing of his wife. She had believed that the facility’s protocols, including a daily check-in system where residents pressed a button in their rooms each morning, would ensure his safety.
Boorn expressed her disappointment in the facility’s failure to follow through on their safety measures, emphasizing that staff were supposed to reach out or physically check on her uncle by 10 a.m. daily. She stressed that despite attempts to contact McClellan and the absence of button presses, no one physically checked on him for four consecutive days.
In light of this incident, Boorn decided to speak out to alert other families with loved ones at the facility. She urged them to remain vigilant and ensure their family members were being properly cared for. Boorn’s intention regarding potential legal actions against LifeStream remains unclear.
