“Man Fatally Struck by Train Due to Headphones”

A man tragically lost his life after being struck by a train at a level crossing in Formby. The incident occurred on August 24 last year when 29-year-old Sam Dudley, who was out for a run, entered the crossing while wearing headphones. Despite warning signs and a traffic light system indicating an approaching train, the man was hit as he attempted to retreat upon realizing the danger.

Footage from the inquest revealed that Sam ignored the red light and klaxon signals, ultimately leading to the fatal accident. An investigation found that his phone, displaying a notification about high music volume, might have contributed to his lack of awareness. The inquest report confirmed the crossing was properly maintained and operated, with no faults identified in its management.

The coroner, Anita Bhardwaj, concluded that Sam’s distraction from loud music likely caused him to miss the warning signals. Although no direct faults were found with the crossing, the coroner highlighted concerns about the positioning of warning signage, suggesting clearer visuals at the entry gate to improve safety awareness. She emphasized the importance of combining visual and auditory cues for effective safety communication, especially in situations where individuals may be distracted by devices like headphones.
