Tube operators have received a caution to be more vigilant following the tragic death of a pensioner who was fatally struck by multiple trains after being mistaken for an inflatable object. Brian Mitchell, 72, fell onto the tracks at Stratford London Underground station and was fatally injured by passing trains before being noticed by station staff. The incident occurred on Boxing Day 2023 and has been deemed preventable by a coroner who highlighted the importance of enhanced attention by train operators, especially when using partially automated systems like the one on the Jubilee line.
CCTV footage revealed that Mitchell, believed to be under the influence of alcohol, moved precariously close to the platform edge before attempting to climb back onto the platform, ultimately being hit by an approaching train. Subsequently, he was run over by two more trains, with a staff member unsuccessfully trying to prevent a fourth train from causing further harm.
The coroner’s report emphasized the need for increased vigilance by train operators, recommending manual overrides of automated systems in the presence of obstacles on the tracks. It was noted that Mitchell’s presence could have been detected and the tragedy averted if the train operator had been attentive. The report also highlighted delays in implementing technology to detect falls onto tracks at the station since the incident, raising concerns about ongoing safety measures.
Transport for London expressed condolences to Mitchell’s family and friends and pledged to address the coroner’s recommendations to enhance safety protocols and prevent similar incidents in the future.
