Tragic news emerged as a 15-year-old boy, described as ‘loving,’ was discovered deceased at home while his mother was away for a brief period. The boy, named Zack Ellis, had been receiving support from various organizations, including CAMHS service at NHS Pennine Care Trust and Oldham Council’s social services in the months leading up to the unfortunate incident.
His mother found Zack’s lifeless body in his bedroom on July 23, 2024, upon her return from dropping off a friend. Investigations revealed that Zack had searched online for information on ‘how to kill myself’ just two hours before his body was found, although no suicide note was discovered in his room.
Described by his mother Kathleen as an ‘awesome’ and ‘loving’ individual, Zack had a passion for art and photography. Despite facing bullying at school, he had a strong bond with his family and friends. Zack had transitioned from being assigned female at birth to identifying as male at a young age.
The court learned that Zack had struggled with mental health issues, including self-harm, which led to his involvement with CAMHS and social services. Despite efforts to support him, including safety strategies and therapy sessions, Zack’s mental health continued to deteriorate.
Various professionals, including mental health practitioners and social workers, were involved in Zack’s care but challenges arose, such as missed appointments and communication gaps between agencies. Concerns were raised about missed warning signs and incidents, such as Zack bringing a blade to college and encountering distressing situations.
Ultimately, the cause of Zack’s death was determined to be by hanging, as confirmed by the pathologist. The case highlighted the need for improved coordination and communication among the different services involved in Zack’s care to prevent similar tragedies in the future.
