“NHS Trust Missed Warning Signs in Nottingham Tragedy”

An inquiry has revealed that a former patient of a National Health Service (NHS) trust, who was also treated by the same trust as the Nottingham attacker Valdo Calocane, was involved in a fatal incident less than a year before the Nottingham tragedy.

According to the inquiry, the fatal incident in August 2022 allegedly involved an individual who had previous contact with Nottinghamshire Healthcare NHS Foundation Trust’s mental health services. The former chairman of the trust acknowledged a missed opportunity to prevent Calocane’s violent actions, which resulted in the deaths of three individuals. Calocane, who had been under the trust’s care from 2020 until his discharge in 2022, carried out a brutal attack on students and a caretaker in June 2023.

The inquiry revealed that another former patient of the trust was responsible for a separate fatal attack in August 2022, although specific details about the incident were not disclosed during the inquiry proceedings. It was highlighted that insufficient actions were taken by the trust’s board concerning this incident.

The former chairman admitted that the trust was informed of the August 2022 incident in September of that year, but an investigation was delayed pending the conclusion of a police inquiry. It was not until May 2025 that an action plan related to the fatal attack was approved. The inquiry also raised questions about the trust’s response to potentially preventable incidents following unsafe discharges.

Calocane, who pleaded guilty to manslaughter and attempted murder charges, is currently detained indefinitely in a high-security facility. The inquiry is ongoing to further investigate the circumstances surrounding these tragic events.
