An inquest revealed that there were missed chances to prevent a tragic house fire caused by a mother that resulted in the death of her young daughter.
In July 2024, Danielle Parkinson shared a poignant 19-minute video on YouTube before taking her and her eight-year-old daughter Iliza’s lives in Huddersfield, Yorkshire. A second child survived the fire.
After a two-day inquiry at Bradford Coroners’ Court, assistant Coroner Caroline Chandler determined that the mother had committed suicide and unlawfully killed her daughter. The coroner highlighted missed opportunities in the days leading up to the tragic event.
Danielle, who had a history of mental health issues, sought help at Huddersfield Royal Infirmary’s A&E department days before the incident due to suicidal thoughts. Despite receiving mental health care, the well-being of her children was not thoroughly considered, and safeguarding measures were not implemented.
During the inquest, Farah Farooq, a psychiatric practitioner, testified that Danielle had seemed positive and discussed plans for a new job during their last meeting on July 29. Farooq mentioned Danielle’s previous struggles and discharge from mental health services shortly before the fire.
Farooq revealed that Danielle had a history of suicidal thoughts but had considered her children as a reason to live. Danielle had expressed enthusiasm about summer plans, including taking her children to watch a Shakespeare play at a theater.
The inquest also mentioned a report by the mental health trust indicating that the children’s perspectives had not been adequately acknowledged. The trust pledged to enhance its services based on the findings from Danielle’s case.
Iliza’s father, Gary, fondly remembered his daughter as always cheerful and enjoying school and time with her loved ones. The family expressed deep sorrow over Iliza’s tragic death.
Danielle’s final message on YouTube, where she disclosed her struggles with bipolar disorder and depression, was played at the inquest. In the video, she expressed her pain and apologized to her friends and family.
